Waiver of Liability

Note within the below form whereas written “Name” refers to the Client.

I, the Client, release Crystal Helix Healing LLC and Yoga Instructor Brittany Travis from any responsibility and/or liability concerning the application, processing, and/or consequences of Hatha Yoga or Energy Medicine Yoga® also known as EMYoga® that I, the Client, elected to participate. I, the Client, consent to have the above described yoga services of my choice applied. Understanding the risks of yoga, I, the Client, release Crystal Helix Healing LLC and Yoga Instructor Brittany Travis as agents harmless against any and all liability, damage, and/or expenses arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) or transmission of a communicable disease that I, the Client, might incur as a result of the service provided in any class or workshop and I agree to voluntarily participate understanding these risks and their outcomes. I, the Client, also affirm that understanding the above described activities that I, the Client, am healthy enough to participate.
